Was reading dynamic nymphing (great book so far) and he mentions that he brings two rods on the river, pre-rigged. Anyone else do this? If yes, what do you rig both rods with and how do you carry the second rod so it is not in the way?

I carry an extra rigged rod in one of Dan Blanton's "Rigged and Ready" rod bags. The bags have one "D" ring. I add a second and clip a strap to the rings and sling the rod across my back.
The bags are a available direct from Dan (contact info on his site) and from Sea Level Fly Fishing. They may be available from other sources. I like the bags so much I have 10. When fishing from a boat it is not unusual for me to have a half dozen rods rigged and ready in these bags. When float tubing I carry my extra rods in these bags too.
No. I just carry sufficient tippet material to adjust my leader to suit, and a spare reel spool. I normally fish with a floating line, and have a sink tip on the spare spool. Never saw the need to carry two rods.
I will sometimes carry a Tenkara rod and another rid and reel. I typically don't carry two rod and reels. I do carry spare leaders, tippet, and a spool with a sinking line.
I almost always carry a 3 wt. rigged with a dry fly - dropper and a 5 wt. with a nymph rig on it when float tubing, which what I mostly do.
