Sounds like a couple of vendors that I've done business with and will never again. The boards seem to welcome the ++++ comments about a vendor but not the negatives. We need a place we can rant and rave about poor or non service.

Last and final update...
The vendor has responded with an overwhelming apology for not getting back to me sooner and is senidng a couple of new tips just for the hassle. He also told me that he doubted if he could determine if it was the wrapping that could have caused the break or not. So at this point, I can only share a couple of things with you, 1.) I don't know if I broke it wrapping it or if it was a defect; and 2.) I will definielty buy from this vendor again because in my opinion he did more than the right thing.
You say you were wrapping a guide when the blank collapsed. It is possiable for a builder to pull so much tension on the thread that they will actualy draw the foot of the guide into the blank and compromise the hoop strength of the blank and thus cause the situation you have discribed. I cant say its common, but its not unheard of, been there done that.
