It's a forecast blank from Batson Enterprises. You can find it in their 2005-2006 online catalog on page 11 at:

I have built a number of Forecast blanks and they perform well for the little money that they cost. They are available thru Hook & Hackle in 2-piece ($25.60) and 4-piece blank ($36.60). I believe the 3-piece was discontinued.
