I am using, when this happens, Mustad hooks.
I do try to keep everything perpendicular, but once in awhile it slips under with the thread either getting nicked or breaking.
Doesn't happen too...

I occasionally get the thread caught in the hook eye with the resulting problem(s).
I have tried a thread "dam" right behind the eye, but that makes for a big head on the finished fly. Have tried...
